Completing the DigitalStakeout Scan Onboarding Form
To activate your DigitalStakeout Scan service, you’ll need to complete the DigitalStakeout Onboard Form. This form ensures we can begin monitoring and classifying threats relevant to your online presence.
This guide explains each section of the form, what’s required, and how the data will be used.
🧾 Why This Form Is Required
DigitalStakeout uses the information you provide to:
- Configure your monitoring profile
- Discover data tied to your name, emails, usernames, and other indicators
- Tag and classify digital threats affecting your digital footprint
Note: The form also includes an authorization for content takedown. While this is not used for Scan, it is required if you later upgrade to Protect. Completing it now avoids delays in the future.

DigitalStakeout Scan includes:
- Online Threat Detection
- Data Broker Discovery
- Social Risk Detection
- Data Breach Discovery
- Risk Classification
- Access to Console & Dashboard
- Email Alerts (Sev 1 Threats Only)
📋 What You'll Need to Provide
1. Client and Account Info
- Client Name – Pre-filled with your organization
- Account Number – Unique ID for your subscription
2. Personal Identity
- Legal Name – Full name including middle name (enter “NA” if none)
- Public-Facing Name – How your name appears online (social media, forums, etc.)
- Maiden Name – Optional, but helpful for historical records
3. Addresses
- Current Address – Required for accurate discovery
- Second Address – Past residence or alternate location (strongly recommended)
4. Other Residents
- Add names of any other adults (18+) at your address who might be linked to your data (utility bills, shared accounts, etc.)
5. Contact Information
- Date of Birth
- Personal Email
- Work Email
- Cell Phone
- Home Phone (optional)
6. Unique Markers
Add known identifiers that are highly specific to you:
- Screen names
- Secondary emails
- Phone numbers
These help improve signal accuracy across the open web.
✍️ Authorization Section
At the bottom of the form, you’ll be asked to:
- Approve content removal on your behalf
- E-sign your name
This authorization is only applicable to the Protect service. If you are using Scan, we will not act on this permission.
However, completing it now avoids any rework if you upgrade to Protect later.
📤 Submitting the Form
Once submitted, DigitalStakeout will:
- Review your data for completeness
- Activate your monitoring profile
- Email you once your console and dashboard access is ready
